Select one substance covered within this course and develop
a comprehensive Psychiatric Nursing Case Study for a theoretical person who is
using that substance. Write a history to provide your faculty with an
understanding of the theoretical patient that you will be basing this study on;
this should include the patient’s personal history, history of substance use, family
history, medical, and psychological history. Because this is not a real person,
you can assign a name and there is no need to refer to your theoretical patient
by initials only; however, do not select the name of anyone you know personally
or have met professionally. Your case study should include the following:
1. Assessment and screening tools used to determine
where your case study client is along the continuum of substance use. Include the DSM-5 diagnostic criteria for
substance use disorders to support your judgment. Provide two substance
assessment/screening tools appropriate for the substance and include the
rationale for your choice.
2. Bio psychosocial aspects including neurobiology
and genetics of substance abuse disorder; and the effect of the substance on
the brain (neurotransmitters) for the substance chosen.
3. Detoxification management from the chosen
substance for your client. Be sure to include: medical management, nursing
care, physiological and psychological events during detoxification, and
potential complications during detoxification. Include a treatment plan post
detoxification (short term). Develop a relapse prevention plan (long term).
Describe your plans and what they would include based on the individual needs
of your patient. Use strategies that are based on evidence.
4. For your chosen substance, create a care plan
for your client who has a history of substance use disorder with your chosen
substance. Identify three nursing diagnoses for this client, including five
nursing interventions for each diagnosis. Provide a rationale for each of your
diagnoses to illustrate how your choice relates to your client and the specific
substance.
The substance I have chosen is alcohol.
