Look at the project, logic model, and implementation plan, attached below, and create 6 pages evaluation plan. It must include the following and reflect goals and objectives.
• Process Evaluation (Formative) (2 pages): Describe the plans for evaluating the fidelity of the program implementation. Here are a few key questions to answer:
o How will you measure if the program was implemented appropriately?
o What data will you collect for process measures? How will it be collected?
o How would you use the process evaluation data to improve your program?
• Impact Evaluation (Summative) (2 pages): Describe the plans for evaluating the immediate impact of the program. Here are a few key points:
o Describe the measurements and data needed to determine if there are any immediate behavioral changes as a result of your program. Note that changes may occur in different ways (e.g., knowledge, attitude, behavior, environment, etc.).
o Will a control group be used in your program? If not, explain how you will determine that it was your program that initiated behavior changes in the population. Remember, only use a control group if it is feasible to do so. Do not simply state that you will use one if it is not possible. If you do use a control, explain how you plan to evaluate changes in both the control group and the intervention group.
• Outcome Evaluation (Summative) (2 pages): Describe the plans for evaluating the overall impact of your program on the quality of life in your population. Here are a few key points:
o What are the long-term outcomes anticipated as a result of your program?
o How will you measure sustained behavioral changes in the population?
o How will you measure if the program has delivered lasting changes in health behaviors such that it impacts health status?
o Describe how you will measure the cost-effectiveness of the program. Although you are not using an actual budget for this project, at minimum, describe how you will determine if the benefits of a program are worth the costs.
