examTITLE: A Case Study Regarding José
Date: March 1
José is an RN working 7am-7pm shifts on a med-surg unit four days per week.
He stays up past midnight watching movies to unwind and then has trouble waking up each morning at 6am.
He has been late to work the last 3 of 4 shifts.

Title: Step 1
Step 1: Develop an Aim statement (or measurable goal) and the Plan.
The Aim statement includes what, when, how much, and for whom (IHI, n.d.a). The plan includes the details to improve the situation and problem.
Aim Statement
José will limit watching movies on work nights in order to be in bed by 10pm, feel rested upon awakening, and not be late for any shifts by March 14.
Plan
Describe the situation and problem that needs improvement.
José is an RN working 7am-7pm shifts on a med-surg unit four days per week. He stays up past midnight watching movies to unwind and then has trouble waking up each morning. He has been late to work the last 3 of 4 shifts.
Describe the change that should be implemented.
Assuming the trouble waking up and being late is caused by staying up late, José needs to stop watching movies past midnight and get to sleep at a reasonable hour. He is currently getting 6 or fewer hours of sleep each night
Describe the predicted outcome.
José will get 7- 8 hours of sleep on work nights, wake up feeling rested, and not be late for work.
Identify who will be involved in implementing the change.
José
Identify who will evaluate the change.
José
Describe how the outcomes will be evaluated.
José will use a journal to record hours slept, feelings when he wakes up, and time he starts work.
Identify when this implementation will begin.
March 2
Identify the target date for completion.
March 14

Title: Step 2
Step 2: Implement the Plan
Note how the plan is progressing and if there are any unexpected events
Do
Describe the implementation in detail.
Each night prior to a work day, José watched a TV show instead of a movie. He recorded what time he went to bed and got up in the morning on a notebook that was on his nightstand. He also recorded how he felt every morning. He used an app on his phone to track what time he arrived to work.
Describe any problems or unexpected events that occurred during implementation.
On March 10, José had car trouble and was late for work.

Title: Step 3
Step 3: Evaluate the Outcomes
After the target date, examine the outcomes of the implemented plan.
Study
Describe in detail the outcome(s) of the implementation.
Of the eight shifts José had during this time, he was only late for one shift. He limited his television watching, was in bed by 10pm every night prior to a work day, and woke up at 6am feeling rested. He was late for work one time but it was not related to his fatigue.
Describe how the problem was improved or not improved.
The problem was improved since the plan helped José get more sleep, feel more rested, and not be late for work.
Identify whether the goal was met by the target date or delayed.
The goal was met by the target date of March 14.
Describe how the actual outcome compared to the predicted outcome.
The actual outcome was similar to the predicted outcome. The event of the car trouble causing José to be late was not expected.

Title: Step 4
Step 4: Consider the next steps.
If the plan was successful, there may be no changes but a plan needs to be created to maintain the progress. If the plan needs modification, revisions are needed before the next PDSA cycle. Both plans need to be detailed and realistic.
Act
If the problem improved, describe how the change will be maintained in the future. If the problem was not improved, describe how the plan will change for the next PDSA cycle. State at least two actions.
Since the problem was improved, José will have to maintain this change. José has selected a few television series that interest him and help him relax. He has set an alarm on his phone for 10pm (bedtime) and 6am (wake up time).ple:
