You are a healthcare service coordinator for a health insurance organization. You have been tasked with coordinating healthcare services for a new client, a 74-year-old man.
A review of his past medical history includes the following information:
He takes daily cardiac medications to treat atrial fibrillation and anticoagulants to prevent blood clots. He is a widower who lives alone in a two-story home in a rural community. He lives independently and does not need assistance with activities of daily living or with managing medications. He drives himself to his appointments and has been active in the community.
His 48-year-old daughter lives nearby with her husband and two children. She checks on him by phone every day but only visits in person weekly due to her busy work and family schedules.
The clientâs medical history describes the following incident that occurred two weeks ago:
When the man did not answer the phone one evening, his daughter decided to check on him. She found him lying in the bathroom on the second floor, trapped between the toilet and bathtub. He was lethargic and unable to answer her questions, and he had a laceration on the right side of his forehead. She called for an ambulance, and he was brought to the emergency room in his local community hospital. He was treated for his head injury, and a CT scan showed bleeding in his brain tissue.
In his neurological evaluation, he was unable to move his left arm or left leg, and as he regained consciousness, it was determined that he had left-sided facial droop.
His community hospital did not have a neurologist on staff, so he was transferred to a larger hospital in the nearest metropolitan area 45 miles away for a higher level of care. He is currently admitted in the neuro-ICU unit.
