Gather data from a secondary data source Community Demographics -For this subsection, briefly talk about your chosen communityâs general demographic characteristics then display the data in a table. Initial data for a community assessment is also available through resources like the MA Department of Public Healthâs Population Health (Statistics) Neighborhood Data In this subsection, you talk about the specific health factors you have prioritized and how they are distributed across the community based on the importance because of the severity or some other measures (e.g. level of risk, prevalence, incidence, or some other objective measure based on using the public health sciences). Also, display the data in a table or provide image(s). This section is not the same as displaying the entirety of your data that you gathered in the Community Demographics section from your secondary data sources. You are stratifying your data by specific factor(s) of interest and bringing attention to the inequities and disparities of your specific health issue that ties back to your original goal of the community health profile.
