Identify a community & prepare Community Health Profile Health issue: Teenagers Vaping – Community: San Bernadino, California In the first paragraph please identify the community of interest, a chosen health issue, goal and purpose of the community health data brief. For example, are you trying to discuss the prevalence rate and associated risk factors of vaping use among teens in the area. In the second paragraph you must geospatially indicate what area you are focusing on and provide a description of the area. For instance, you should be able to, at a minimum, screenshot the area of interest from using Google or Apple maps. There are other ways to capture and highlight the geographic boundaries of your area of interest as well by using Google Maps or Google earth. Once you have a picture, write a brief description in the text of the area you are focusing on. Reference(s) must be included (APA format). Label your images e.g. Map 1. Gather data from a secondary data source Community Demographics -For this subsection, briefly talk about your chosen community’s general demographic characteristics then display the data in a table. This is where you gather data from a secondary data source. For instance, you can use the U.S. Census/American Community Survey or another secondary data source for your community of interest. Initial data for a community assessment is also available through resources like the MA Department of Public Health’s Population Health (Statistics) Neighborhood Data In this subsection, you talk about the specific health factors you have prioritized and how they are distributed across the community based on the importance because of the severity or some other measures (e.g. level of risk, prevalence, incidence, or some other objective measure based on using the public health sciences). Also, display the data in a table or provide image(s). This section is not the same as displaying the entirety of your data that you gathered in the Community Demographics section from your secondary data sources. You are stratifying your data by specific factor(s) of interest and bringing attention to the inequities and disparities of your specific health issue that ties back to your original goal of the community health profile. It is more important to describe the inequities by the specific health factor(s) of interest. Reference(s) must be included (APA format). Label your tables and images e.g. Figure 1. Analyze data to write Summary & Conclusions section In this subsection, you are drawing your conclusions based on the available data you were able to obtain from secondary sources. You want to emphasize the goal of this community health profile. References and Other Resources Indicate your references and other resources (APA format), which must be credible.
