This week’s assignment focused on Essential III: Quality Improvement and Safety & Essential IV: Translating and Integrating Scholarship into Practice. We were instructed to describe what inter-professional collaboration you connected with and to describe how you incorporated that insight into planning your project, while also addressing the above two mentioned Essentials.
For my practicum project, I have collaborated with the VA Community Care (VACC) Nurse Manager, the VACC nurses and staff who manage the category of Home Health, the VA Social Work Service (social workers), and the Primary Care nurses. I also want to incorporate collaboration with the Home-Based Primary Care nurses and social workers, VACC contracted home care agencies, and veteran caregivers. It is a multidimensional collaboration of inter-professional, family and community participants who play a crucial role in assessing and managing veterans’ care. I also want to incorporate individual and interdisciplinary involvement of addressing and influencing practice problems and patient outcomes, directly and indirectly.
The topic of my practicum project evolves around the concepts of ethics and quality, along with developing a performance improvement measure to evaluate and enforce ethical and quality standards for our geriatric veterans receiving non-skilled home care. I plan to monitor, analyze, and prioritize these outcomes. My goal is to disseminate my research results within the service of VACC and our local VA Medical Center, resolve the practice problems of unethical and substandard care in our veterans’ homes, and utilize my research outcomes to promote and sustain the healthy physical/psychological well-being and safety of our geriatric veterans within their home environments. I want to promote effective and efficient care to reduce the instances of fragmented care, utilizing quality improvement tools or measures. I will keep in mind the 2022 Home Care National Patient Safety Goals to prevent infection, prevent injury and identify patient safety risks (2022 Home Care National Patient Safety Goals, 2022). I want to be an advocate for the caregivers and the care recipients (veterans), as they are both integral, dynamic players within this context.
