Baccalaureate RN Role in Delegation
The registered nurse has many roles and duties when it comes to delegation. The nurse has the responsibility to complete patient care assignments and to perform them correctly. The nurse has the authority to delegate assignments that are within the scope of practice to the delegate; however, the nurse maintains accountability that the delegated task is completed and done correctly (Barrow & Sharma, 2021). According to the article by the American Nurses Association and the National Council of State Boards of Nursing, the nurse must follow the Five Rights of Delegation: The right task, under the right circumstances, to the right person, with the right directions and communication, under the right supervision and evaluation. Of these five rights, providing the right directions and communication is in my opinion the most important as it can result in patient harm despite the other rights being followed. According to Barrow and Sharma (2021), when the nurse delegates, they need to “use direct, honest, open, closed-loop communication to encourage teamwork and safe task performance” (p. 2). Unclear directions that are open to interpretation allow for greater risk of mistakes made by the delegate.
Experience in Delegation
At Surgicare of Hawaii, we have two medical assistants on the pre-op and PACU units. When I first started working, I had asked my supervisor the scope of the medical assistants as each state has different laws. In Idaho, medical assistants are allowed to start IVs whereas Hawaii they cannot. If I had delegated this task, I would have violated the five rights of delegation. After reviewing my states scope of practice for medical assistants, I can safely delegate vital signs upon admission, intake questionnaire and gathering of IV supplies. I delegate these tasks when we are particularly busy in pre-op and there are not enough RNs to complete the full intake process with each patient. This speeds up the rate at which we prep patients for procedures to keep surgeries on schedule while maintaining patient safety and safe delegation.
Resources that Support the Delegator
A resource that would support me, the delegator, is a safe delegation class provided by through my facilities monthly continuing education requirements. As I had stated earlier, I was not up to date on what tasks my team members can and cannot perform by law. On my own, I researched Hawaii’s scopes of practices for different healthcare providers to ensure I was delegating safely. I believe that it is also my facility’s responsibility to educate their staff on safe delegation. According to an integrative review performed by Beckett et al (2021), “In 6 of 7 papers, nurses who received education about delegation reported improved delegation knowledge and decision making… two articles reporting reduced patient falls and one paper reporting fewer missed and delayed tasks.” (p. 257

The Joint Commission Emergency Preparedness website addresses up to date standards on emergency management to help organizations prepare for the event of an emergency. This provides organizations with instructions on specific roles and duties to be carried out in order for an effective emergency response.
Change in Delegation Role in an Emergency Situation
According to the Hawaii Department of Commerce and Consumer Affairs (2020), Hawaii Nurse Practice Act Chapter 457 states there is no change in the nurse’s delegation roles during emergencies. All delegations, during emergency or not, must be followed in accordance with the five rights of delegation, delegation-decision making tree and the continuum of care framework (p. 16).
