The interventions in those patients will include
weight loss, diet (DASH and Mediterranean), blood pressure check at home, exercise,
avoid alcohol and cigarette, provide education in Spanish, low cholesterol and
triglycerides, 1. Short-term
of 6 months: Decrease systolic BP and BMI, helping decrease cardiovascular
disease (CVD). 2. Long-term of 12 months: Decrease of
systolic BP achieving normal BMI leading to fewer pharmacological interventions
and less CVD incidence in most of the patients. Process evaluations: · Evaluate
the patient weight and BMI every three months. · Monitor
the patientâs BP log to assess a decrease in systolic BP.
· Assess
patient cholesterol, HgA1c, and triglycerides.
Outcome evaluations
· Evaluate every Hispanic patient who
presents to the clinic with hypertension regardless of the hypertension stage
every three months to assess for results. · Evaluate every adult Hispanic
patient 12 months after initiating intervention to assess for results
compared with baseline.
