A 7 year old Rwandan female with a history of chronic infection is taken to a local health clinic by her mother after a recurrent episode of severe upper respiratory infection. Per the mother, She encounters these infections at least seven times per year, particularly when her father returns from Egypt (her father owns a business selling tapestries in Cairo). Her mother states that she and her family have lived in a small village just outside of Kigali for the past 6 years and that she has never forced her daughter to receive vaccinations from local health clinics or incoming mission workers because she does not believe that they are safe.
This time however, in addition to the upper respiratory symptoms, she is also experiencing the growth of a large mass just below her jaw. This mass initially began as a very small, painful nodule on the left side, but it has grown substantially within the past few weeks, at times suppressing her airway. Of note, the young girl has also experienced significant weight loss, losing approximately 25 pounds with in the past three months. Also, the little girl was actually born with kwashiorkor and resultant albumin deficiency.
Concerned with these findings, the director of the health clinic suggested that the patient leave the clinic and immediately go to the larger hospital located in Kigali.
The patient went to the hospital with her mother, and she underwent an emergent bone marrow biopsy; The histologic results lot of The histologic results from the bone marrow biopsy were revealing of enlarged lymphocytes with very dark nuclei; serologic testing of the biopsy samples were revealing of an 8–> 14 translocation, resulting in the formation of an oncogene which is diagnostic for Burkitt Lymphoma.
The girl underwent conventional chemotherapy, and recovered very well.
