Capstone Final Paper Overview The final paper is a comprehensive review of a specific population group that has a specific health problem. It includes a needs assessment, data, evidence-based interventions, measurable goals and outcomes, collaboration with a care service agency/organization, and the role of the nurse as an advocate and change agent.
Write the paper in APA format 7th edition. Use level one headings (upper/lowercase, bold font, centered), A minimum of 10 resources is required, including 5 scholarly references (from peer-reviewed journals). Include all DOI’s in APA 7 format if applicable. A course text may be included. For web sources be sure to use sources that end in .edu, .gov, or .org. Web sources from .com sites are not acceptable, including Wikipedia, WebMD, etc. Turn it in score of no greater than 15% for plagiarism. Introduction to the project/paper Begin the paper with an introduction of the identified needs of the community and specific patient population. Based on your assessment and research findings, state the facts, statistics, and urgency to provide care and interventions to this population. Identification of Target Population and Needs Assessment
What health issue, problem, social determinant, or disparity in health outcomes is of concern to you as a new nurse and care provider in a community? What gaps in knowledge and care do you see as a possible cause for the health issue? What does the health data and literature tell you about the health issue? Include a description of the social determinants that are contributing factors. Refine your population and problem and compare your findings of this problem to local, state, and national data. Consider how the nurse might help them avoid further development, or disease progression (primary, secondary prevention measures). To start your search and interest you might look at your community assessment, or look at the information in your local public health department websites, the CDC, (CDC Wonder databases and Healthypeople 2020, 2030), and other evidence-based websites that the CDC supports.
Refine the health data about the issue that affects your population group, using scholarly professional literature to support your ideas about the population at risk. Look for data at the local, county, or state public health websites. Some examples of health data you might consider are epidemiologic information related to health conditions, reproductive outcomes, causes of death, vital statistics, socioeconomic data including poverty and/or educational levels, quality of life issues, and or lack of access to health care due to health insurance or access to providers. You should compare the data with state and national trends to fully understand the extent of the problem with the population.

Identify at least 3 measurable goals for the patient/population that are SMART (Specific, Measurable, Achievable, Realistic, and Time-bound). Identify any obstacles or barriers to assisting the patient/population to achieve the goals in your discussion. These would include the social determinants of healthâeconomic, education, physical environment, etc.
Consider nursing interventions for key health concerns in your population. Develop evidence-based, culturally appropriate, measurable (SMART) interventions to help your population members maintain an optimal state of health. Include the 3 levels of prevention (primary, secondary, and tertiary) in your goals. Is there a possibility of changing the behavior or social determinants of health to improve conditions? How would you advocate for this? What type of change might be expected? How will you know when a change has taken place? State what these are and how genetics and culture may have a profound effect on the health outcomes for the population.
Consider the cultural considerations that might inform your approach to caring for this population. How will this influence the education, interventions, and care planning for the population?

Care coordination is an integral part of the continuum of care. Identify the agency/organization and briefly describe their services to this specific patient/population. This may be a local, state, or national profit or non-profit organization that provides services to the identified health issue and patient/population. An example might be a hospice, Senior Center, Head Start Center, shelters, meal services, American Heart Association, American Cancer Society, American Lung Association, Alzheimers Assoc etc. Briefly include the services they provide, the number of persons served and how the organization/agency is funded.

The community that I have decided to focus on is San Francisco, which is located in California. Teenagers make up the bulk of San Francisco’s population that is considered to be vulnerable. Unfortunately, teenagers have a higher risk of contracting sexually transmitted diseases than adults do. This is due to a number of factors, including a lack of early sexual education, substance usage, and the fact that adolescents find it awkward to discuss sex with medical experts. Between the years 2013 and 2017, there was an increase in the number of adolescents who were diagnosed with sexually transmitted infections such as gonorrhea, syphilis, and chlamydia (San Francisco Department of Public Health, 2019). Teenagers are at a disadvantage since they are at a moment in their lives when they are undergoing challenging stages, and their bodies are also undergoing changes, which makes it difficult for them to know the best ways to cope with these challenges. They are going through a variety of transformations, including emotional, physical, and social shifts, all of which have a significant impact on the decisions that they make. Teenagers are at risk for contracting sexually transmitted infections because, in addition to wanting to explore the newly emerging feelings and changes in themselves, they also lack the knowledge necessary to properly protect themselves from those infections. This makes adolescents more likely to engage in sexual activity without appropriate protection. They tend to have less access to and utilization of sexual health services, which can help protect them against sexually transmitted illnesses. Additionally, they are more likely to be interested in experimenting with drugs. When it comes to the education of adolescents on the prevention of sexually transmitted infections, parents and schools play a significant role in the behaviors that adolescents develop regarding their sexual health. Adolescents are particularly vulnerable to the long-term health effects of sexually transmitted diseases, which is why it is critical to promote frequent screenings for these diseases (Shannon & Klausner, 2018).
