705.3.4 : Patient Care Improvement
The graduate interprets quality assessments and prioritizes patient care improvement needs.
INTRODUCTION
In this task, you will use a tracer methodology to track a patientâs care in order to evaluate the healthcare organizationâs systems of providing care and services for a readiness audit. This methodology also makes it possible to assess the healthcare organizationâs compliance with Joint Commission standards. This is part of the organizationâs compliance with delivering safe, quality healthcare.
You will examine data for a patient that is found in the attached âAccreditation Audit Case Study â Task 3 Artifactsâ and find any trends, patterns, and problems. Once these have been identified, you can remediate the concerns.
REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.
You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
A. Review the tracer patient information from the attached Accreditation Audit Case Study – Task 3 Artifacts and do the following:
1. Discuss an outstanding patient care issue for the organization made evident by the tracer patient.
2. Develop a corrective action plan to address the patient care improvement needs for the organization using a keyword search in the Joint Commission electronic manual.
B. Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.
C. Demonstrate professional communication in the content and presentation of your submission.

Task 3
You will wish to listen to the Cohort Walk Through Lectures in the Tips section, this is the most complete resources and will allow you to move quickly with less stress. You may also request the slides from the lecture as most materials you will need are there for easy access form Austin.arenz@wgu.edu. This task is typically the shortest and often creates some student concern as they believe that they have forget something. This area introduces us to two real world concepts Tracer methodology (which you are seeing the tail end of in Task 3) and the Patient summary sheet which is the worksheet the TJC uses to list areas of concern. You will need the artifacts from task 3 assessment area
This goes very quickly after watching the lecture and is typically finished in an evening or so. You are to look at the FSA sheet and pick an issue. The one they are really centering on is the event that almost killed the patient, but you can pick one of the other areas on the FSA sheet.
Do a quick search of the EP in the electronic TJC manual. Use the search word from your selection. If you are doing the main incident that nearly killed the patient because the patient was not evaluated during the typical H&P within 24 hours use the search words History or physical (history and physical together will not work).
In the second section you will say what you plan to fix the issue-ie safeguards that make sure patients receive their H&P quickly like requiring a shift leader signature on all new patients before a shift is dismissed and so on.
Again, in this section you will be required to say who is responsible, when is it due and what metrics to use.
