Each student will assess and develop a plan for community wellness. A community wellness assessment scoresheet is provided. Use the scoresheet to record your answers and keep notes. At times, you may need to actually go to into the community to gather the information to answer the question. Tally the overall score.
Next, based on your finding, develop a community wellness plan to address the gaps you noted in your assessment. For example, does your community need to develop bike trails for the public? If so, what steps can you take to inform community leaders and stakeholders. What organizations in your community can your partner with to make small change towards a common goal?
At minimum, you must make at least 4 recommendation for community wellness improvement. Use scholarly resources to support your responses. You may have to find additional resources online if a gap was identified we did not discuss in class.

Make 4 recommendations that should include arguments why these specific recommendations would improve community health. These recommendations should integrate with class readings or additional scholarly resources. You also need to think about things like community stakeholders and how you would actually go about funding or organizing these recommendations.
