Hallmark Assignment: Community Project Presentation Overview
Note: You will be asked to submit a Community Project Presentation in Week 14 of the semester, but it is never too early to refresh your presentation design skills.
The goal of the Community Project is to have an opportunity to experience the role of the Community Health Nurse.
The project involves three separate steps – we will provide worksheets that will ensure you are identifying the proper criteria to be successful:
1. A thorough community assessment. You will have an opportunity to complete a windshield and researched assessment on your community. During the assessment you will provide a clear picture of the community you chose. This will include:
o Community Information
o Community Demographics
o Health Data
o Social Data
1. A community teaching proposal focused on prevention. The research completed in the community assessment will identify prevalent chronic, infectious and priority community concerns. You will create a one stop – one hour teaching session for the community. This is only a proposal and not “hands on.”
2. The last step is a disaster assessment of your chosen community. An opportunity to assess the readiness and resources that are available in case there is a disaster.
Final Presentation
1. Develop a Power Point presentation with a minimum of 20 slides (not including the title slide or reference slides).
2. Slides
o Key points (maximum 8 words x 5 lines).
o Include images, graphics and professional in appearance
3. Detailed presenter/speaker notes
o APA Guidelines: Full sentences, in-text citations, paragraph format
Thesis Statement
• A thesis statement is a sentence that states the topic and purpose of your
paper.
• Should show exactly what your project will be about
Comes at the end of your introduction
• Sample:
• This presentation will discuss the role of the community health nurse in XXX county.
This will include a community needs assessment, identification of a priority health
concern and a plan for community education, and a disaster assessment.
Rubric criteria

• 45 to >40.0 pts, Exemplary
• Appraises/ Analyzes a relevant topic and a thesis that provides direction for
the content that is engaging and thought provoking.
• The thesis clearly and concisely states the position, premise, or hypothesis
and is consistently the focal point throughout the presentation/paper.
• Demonstrates a sophisticated examination and deduction and careful,
critical analysis of the research topic and thesis (argument).
• Presents differing perspectives, considers integration of opposing positions,
and draws original and thoughtful conclusions with future implications.
• Decision Making and Evidence Based Practice
• 40 to >36.0 pts, Exemplary
• Provides distinguishing, and accurate evidence base analyzing and
integrating the main concept beyond the required (2) research sources with
at least 1 source from a periodical database.
• Research sources are relevant, accurate, current (less than 5 years old)
reliable, reflecting and validating the strength of the content.
Writing and APA Formatting
• 30 to >28.0 pts, Exemplary
• Style and voice are appropriate to the given audience and purpose, but also show
originality and creativity.
• Word choice is specific, purposeful, dynamic, and varied.
• Free of mechanical and typographical errors.
• A variety of sentence structures and effective figures of speech are used. Writer is
clearly in command of standard written, academic English.
