I have attached the instructions below. This is my project idea and what the needs assignement will be based on
Post-operative mortality rate continues to increase year by year. Infections and the lack of follow up care is the major contributor to these rates. My focus will be for those individuals who are 60 years or older who do not routinely seek follow up care due to their financial struggles. The most important struggles patients encounter after surgery are the lack of education on wound care, expectations and medication compliance. An integral part of helping these undeserved communities is to implement a system follow up regimen. Implementing a patient education care guide prior to discharge is an effective way to facilitate the patient and the family with immediate information in case of any change. Setting a brochure that depicts patient goals with a time frame is a positive benefit and can help the patient be more involved and cooperative in their recovery journey. Handing out a brochure will not suffice the impact we want from this, therefore having a nurse or physician sit down with the patient and the family and discuss what they can expect every step of the way and providing them with an immediate number so that they can reach you can make a huge impact in the outcome of care.
