1. For any healthcare activity, three performance factors can be measured: structure, process, and outcome. Identify one structure measure, one process measure, and one outcome measure that could be used to evaluate the following hospital admission process: Upon arrival, the patient reports to the hospital registration or admitting area. The patient completes paperwork and provides an insurance identification card, if insured. Often, patients register before the date of the hospital admission to facilitate the registration process. An identification bracelet, including the patientâs name and doctorâs name, is placed around the patientâs wrist. Before any procedure is performed or any form of medical care is provided, the patient is asked to sign a consent form. If the patient is not feeling well, a family member or caregiver can help the patient complete the admission process.
First, we must understand each piece of the measures: structure, process, and outcome. âStructure measures are used to judge the adequacy of the environment in which patient care is provided. Process measures are used to judge whether patient care and support functions are properly performed. Outcome measures are used to judge the results of patient care and support functionsâ (Spath, 2018, p. 44). This model was brought forth in 1966 by Avedis Donabedian. Each of the categories listed above are to be measured separately; but judged collaboratively. Every facility has their own adopted protocol in place for direct admissions, scheduled in-patient appointments, and emergency room admissions.
One structure measurement that could be used for this scenario is how many hours registration personnel are staffed within the facilityâs admitting department. This should be directly followed by ensuring adequate training for emergency department registrars to complete any after-hour ambulatory practice direct admissions. Locally, both major health systems have in place an admitting department staffed full-time Monday through Friday. These departments are staffed from 0800-1700, daily. One of the health systems requires the emergency department registrars to complete all after hour direct admissions; but the other requires emergency department triage registered nurses to complete this process.
The process measure most feasible is two-fold. First, each patient should have a wristband in place. This bracelet contains a variety of information such as: name, date of birth, medical record number (MRN), and a contact serial number (CSN). All of these are pertinent information, but the CSN is the only number specific to this encounter. Second, the registered nurse, typically, will greet the patient upon their arrival to the department. After confirming the patient has a wristband in place, this provider will attempt to locate the patient (dependent on if/or what electronic medical record program is in place). After locating the patient within their system, the patient can then be âbedded,â via electronic bed placement. If the patient does not have a wristband in place the nurse/provider will not be able locate this patient within the medical record software. If the patient is not located then providers will not be able to place this patient in a bed, place orders within the chart, or pull medications from the Pyxis/Omnicell (again, dependent on what the facility uses).
Outcome measures could easily be filtered on the âback-endâ of registration. Often issues are not found until after billing to insurance companies has already been completed. This can become a costly mishap for healthcare organizations. Organizational compliance for ensuring accuracy while gathering and inputting patient information is key. The results of this measure are precisely why patient information should be confirmed and reconfirmed with any movement or procedure. Providing high levels of accuracy are of great importance; but ensuring a timely process is likewise crucial. Patientâs do not want to spend any unnecessary time within the registration process.
Although all healthcare organizations adopt policies and procedures that may vary greatly, basic concepts must be met. First, an organization must adequately staff and educate their team members. Next, maintaining compliance by all team members will reduce potential for error. Finally, instilling the importance of accuracy for the initial process of registration could be lifesaving.
