Disscussion one: It is undeniably true that there exists an academic gap in medical studies due to the few selected resources with ample connoisseurship in the surrogate role of the nurses. This contribution assesses the existent nexus between the Hildegard Peplau medical theoretical contribution, especially in defining the role and the relationship between patients and nurses. Peplau is medical jargon reputable as the “nurse of the century” due to her various discoveries and academic projections that continue to define modern nursing studies and practice in defining the roles and responsibilities of the nurses. Thus, according to Pecanac et al. (2018), who document doctoral dissemination on the interpersonal relationship theory, the surrogate role of the nurses defines the nurse as a substitute or an advocate to the patient and one who the client is well versed with as a teacher, friend, parent, or a sibling.
A deep conceptual analysis of Peplau’s definition of the surrogate role establishes a theoretical ideology encompassing the definition of a nurse’s six main roles- defining their responsibilities as a teacher, resource person, stranger, resource person, leader, and surrogate. Peplau’s definition of the surrogate role of a nurse as an advocate for the patient continues to apply in contemporary society as the established phases of the interpersonal relationship theory between the patient and the nurse continue to apply in contemporary practice. This repertoire concurs with D’Antonio et al. (2018). They document the surrogate role of the nurses as the responsibility where the nurse is mandated to guide the patients to differentiate domains of interdependence, independence, and dependence.
According to Peplau, the surrogate role of the nurses involves first therapeutic communication, which infers to building a personal connection with the patient physically, which allows them to be candid about the body defects. According to interpersonal relationship theory, a therapeutic relationship requires an equal partnership between the patient and the nurse to make mature and valid decisions and avoid making human errors such as missing information on symptoms (White et al., 2014). The sequential phase of the interpersonal relationship commences from the orientation phase, where the patient is unfamiliar with the infection and therefore may omit some information or avoid asking questions due to anxiousness. The following phase is the identification stage, where assume the parent or advocate role where the nurses emphasize to patients to be safe under their hands. Finally, the exploitation phase refers to a stage where nurses provide the patients with the necessary care or optimal health, which may include teaching them to recover their responsibility. These phases continue to define the interpersonal relationship of the medical workers and their patients, especially In APRN nurses who have equal physician rights to decision making – hence establishing Peplau’s definition as valid.
Disscussion Two: The surrogate role ensues as the duty of the nurse to assist the patient in identifying the domains of independence, interdependence, dependence, and acts as the patient’s advocate (Courey et al., 2018). Indeed, the role is relevant to nursing practice as currently experienced. Courey et al., (2018) reveal that under the surrogate role, the patient reflects all behaviors and emotions of his past feeling for their relatives onto the nurse. This is so because the patient is dependent on the nurse’s care. Under such circumstances, the nurse embraces the parental role, ensuring that the patient is always provided with all the necessities and treated like their parents would treat them. However, the client is reminded that such a relationship is only temporary till the treatment is complete. Such reminders help the patient not to assume close attachment to the nurse, which can lead to disappointment in case a certain action is not performed as required. According to Tas et al (2019), under the surrogate role, there needs to be clarification on which areas require the patient’s dependence, interdependence, or independence on the nurse. For instance, in nursing homes or intensive care units, the patients may rely on the care offered by the nurses in which the nurse becomes a surrogate role.
Tas et al (2019) imply that patients that reside in such facilities look up to the nurse as the primary care provider of all their needs. In the absence of such nurses, the patients may not accept help from another individual. More so, it may be difficult for the patients to trust another individual that is not their previous care provider. Courey et al., (2018) state that the surrogate role is relevant to current nursing practice because, under this, the nurse recognizes all the emotions of the patients in which he assists the patient in identifying the similarities and differences. In other words, the nurse acts as a concerned relative that intends to help the patient regain their health.
Moreover, the nurse assists the patient in achieving the required treatment goals. As such, the nurse directs, guides, and offers support to promote the active participation of the client in improving their health. Tas et al (2019) state that the surrogate role involves the nurse helping the patient follow the care plan offered by the medical practitioner. In so doing, the patient’s health greatly improves, and they can also learn the basic ways of adopting a healthy lifestyle. Therefore, as defined by Peplau, the surrogate role is relevant to the nursing practice as currently experienced.
