WRITE A STRUCTURED DIAGNOSTIC REPORT–ESSAY FORMAT
1) FIRST PARAGRAPH: DEMOGRAPHIC INFO–1-2 sentences

2) SECOND PARAGRAPH: MENTAL STATUS EXAM—3-4 sentences
Describe FOUR or more SPECIFIC observations relevant to a Mental Status Exam (appearance & behavior, thought processes & content, mood & affect, intellectual functioning, awareness of surroundings (orientation x3)
3) THIRD PARAGRAPH: PSYCHOSOCIAL FACTORS—2-5 sentences
Describe THREE or more SPECIFIC psychosocial (environmental, situational, and personal) factors that positively or negatively impact your client’s symptoms and daily functioning that will be important to consider when developing a treatment plan for your client and determining their prognosis
4) FOURTH PARAGRAPH: CLINICALLY SIGNIFICANT INFO & DSM-5 SYMPTOMS & DIAGNOSTIC CRITERIA
Describe FIVE or more SPECIFIC, clinically significant thoughts, emotions, behaviors, or experiences, and any other relevant diagnostic criteria with which your client is presenting
“Translate” each of your observations into REAL and SPECIFIC symptoms listed in the DSM-5
Example: “Leigh says, ‘I hear people arguing in my head,’ therefore, she is experiencing auditory hallucinations.”
5) FIFTH PARAGRAPH DIAGNOSIS(ES)
Describe the REAL and SPECIFIC mental disorder(s) listed in the DSM-5 for which your client meets the criteria based on the REAL and SPECIFIC DSM-5 symptoms and diagnostic criteria you have identified
IF the client’s symptoms correlate with a specific pattern of subtype of the disorder, please identify it.
Do this for any comorbid or provisional diagnoses
Specify whether the client’s experience of their disorder(s) and symptoms are mild, moderate, or severe
6) SIXTH PARAGRAPH TREATMENT PLAN
Describe IN DETAIL the BEST treatment plan for your client based on their diagnosis and psychosocial factors (SPECIFIC therapy, SPECIFIC medication, etc., and HOW THEY WILL WORK.)
7) SEVENTH PARAGRAPH PROGNOSIS
JUSTIFY your prognosis (good, fair, or poor) for your client using their diagnosis(es), symptom severity, and psychosocial factors.
FORMAT & SUBMIT THE DIAGNOSTIC REPORT ON YOUR CLIENT
ESSAY FORMAT, do not list information or use bullet points anywhere in your report
