After reviewing this weekâs resources, you will conduct a SWOT analysis of your proposed community health/nonprofit hospital partnership. Refer to your Week 2 critique as you prepare your SWOT analysis, taking into consideration the benefits of your hospital being a partner in community health. Using the template example shown in this week’s resources, recreate it in Word using Tables.
Your report should include an introductory section on the assignment, your discussion of the value of conducting a SWOT analysis, your SWOT findings and your summary which includes your justifications to proceed with forming the partnership. Be sure as part of your analysis that you have identified at least three areas in which your partnership will benefit the community, i.e., reduce unnecessary emergency room visits and reduce the rate of readmissions within 30 days of discharge.
Each cell in your SWOT analysis table should include five to seven key items in bullet form. Embed your SWOT analysis table into your assignment.
