Goals for the first day: My primary goal is to become acquainted with the staff, layout of office, flow of patients and basic office operations so that I am able to be part of the team and not stick out. I also would like to be able to identify at least 1 correct diagnosis on my own.
What do I hope to achieve by the end of the session: I hope to be fully confident examining patients and making a diagnosis or at least putting together a solid list of differential diagnosis.
The practicum procedures and experiences list will act as a guide to help me prepare for encounters. I will have the opportunity to look into items that I am not knowledgeable about before I have the noted experience. I will also be able to seek out different patients and cases that match up to the list so I can have some real world experience with as many items on the list as possible.
During the undergrad rotations, there was not a very high expectation of medical knowledge. They placed more focus on the interaction and day to day tasks that RNs were expected to go through. I also felt like we were there as added help and there was not as much time or effort put towards learning. I ended up doing a lot of CNA type work vs nursing work. Some of my clinical experiences were excellent but there was less input and more listening. During the NP clinicals I expect there to be a lot of listening and a lot of input being solicited from me. I think there will be periods of passive learning, much like there was during undergrad but I think there will be a lot more active participatory learning once I have progressed a little bit.

One significant factor is the mandatory extra shifts and hours that are sometimes put in place at work especially when COVID or Flu number ramp up or census is high. There is not much that I can do to avoid this, but I will communicate with my preceptor in advance when census is getting high and warn him about the potential interference. I know that they operate 6 days per week, so I am confident that I will have the opportunity to make up my hours. My lack of understanding in some areas might pose a problem. I will use the clinical practice guidelines to help fill in the areas where I am lacking knowledge. There are a variety of evidence-based resources that I will likely utilize during my clinical rotations. I will utilize The Practice Guidelines for Family Nurse Practitioners because it is an easy access and easy to navigate reference for treating patients in a variety of settings and across a wide age group. This will assist with common diagnostic criteria and national guidelines. This guide is frequently updated with national standards so I should get the most up to date guidance possible. The clinical practice guidelines on the NIH website also offer a variety of resources to assure that I have solid information available to me to query and treat with. While the NIH sites do not provide as many fixed protocols, they provide a great deal of general guidance that can help recommend a course of action. For quick access, in real time, I will likely use either the Family Practice Guidelines FNP app or similar. This app provides guidance on a smart device or computer to help with clinical decision making.
