Chief complaint
âMy heartburn has been waking me up at night.â
History
A 47-year-old male patient (Greg) presents to the clinic with complaints of intermittent nocturnal gastroesophageal reflux. History reveals that he awakens experiencing burning pain substernally and in the back of his throat. This results in âmy larynx closing downâ and his being âalmost unable to breathe.â As soon as he can breathe effectively, he swallows âa lotâ of antacid and flushes it down with water. The entire episode is very frightening, and he is often afraid to go back to sleep. Because he already has a problem with mild sleep apnea, he is becoming increasingly tired and unable to function at work due to lack of sleep. He now sleeps only in his recliner. He is also concerned about the substernal pain because his father had a myocardial infarction at age 49 and required coronary artery bypass surgery.
He is 5 feet 9 inches tall and weighs 220 pounds (body mass index [BMI] is 32.5), with much of his excess weight carried in his abdomen. He is not a smoker, âoccasionallyâ has three or four beers with friends, and âoftenâ has pizza or submarine sandwiches for lunch with a âdiet cola.â He takes no drugs other than the antacid after a reflux episode.
Assessment
A chest x-ray and electrocardiogram are negative for cardiopulmonary disease, and Greg is diagnosed by history with gastroesophageal reflux disease (GERD).

Case Study #2
A 43-year-old African American male has a history of hypertension, hypertriglyceridemia, and avitaminosis D.
Current medications include ezetimibe 10 mg daily, niacin SR 1,000 mg at bedtime, hydrochlorothiazide (HCTZ)/lisinopril 25/20 mg daily, and vitamin D3 over the counter 5,000 units twice a day. He had a previous testosterone level drawn at another facility that he reports as being low.
He endorses a feeling of fogginess, lack of energy, core weight gain, decrease in erections, and decrease in semen consistency.
Labs reveal testosterone 180 ng/dL (193â836 ng/dL), and thyroid-stimulating hormone/thyroxine, vitamin D, follicle-stimulating hormone/luteinizing hormone, and prostate-specific antigen within normal levels.
1. Is this patient a good candidate for testosterone therapy? Why or why not?
2. What are his risk factors for having erectile dysfunction?
