Purpose: Our group will focus on how diabetes impacts aging adults 65+ and older with chronic illnesses. the challenges of disease management for diabetes and other chronic conditions, improving patient outcomes, and enhancing the quality of life that will support their ability to live independently.
The purpose of this paper seeks to identify what interventions can assist aging older adults living with diabetes, review existing data, identify gaps in care, and provide recommendations for future programs and research.
