Clinical assessment of the patient consists of collecting information and drawing conclusions based on observations, psychological, neurological tests, and interviews to determine the patient’s diagnosis based on the presenting symptoms. Some tools that can be used to gather clinical data include learning about the client’s skills, personality characteristics, cognitive and emotional functioning, and learning about the environmental stressors, and cultural factors before considering the best treatment plan–if needed (Dains, Baumann, & Scheibel, 2015).
According to the class text, the mental status assessment includes four main categories; appearance, behavior, cognitive functions, thought processes, and a mini-mental examination (Jarvis, 2015).
1. Appearance: Posture, Body movements, Dress, Grooming, and hygiene
2. Behavior: Level of consciousness, Facial expression, Speech (quality, pace, articulation, word choice), Mood and affect
3. Cognitive functions: Orientation, Attention span, Recent and remote memory, New learningâthe Four Unrelated Words Test, and Judgment
4. Thought processes: Thought processes, Thought content, Perceptions, Screen for suicidal thoughts (when indicated)
5. Perform the Mini-Mental State Examination or the Mini-Cog
