Examine and describe the process you would follow to assess the organization where you plan to conduct your project for opportunities for improvement, including the identification of stakeholders.
Organizational readiness for change is the measure of the psychological and behavioral readiness to implement change (Puchalski & Straus, 2019). Assessing the organization where I plan to implement the purposed change project can be completed by conducting an assessment to determine the leadership’s capacity to lead change and middle management’s capacity for change. Another consideration is evaluating the organization’s history of implementing change. Part of the assessment is also determining the understanding of the stakeholders of the need for change and their capacity for change. Other factors that should be considered prior to change project implementation are the potential impact of change and the purpose of the change. Stakeholders will be more supportive of purposed changes if they have a clear understanding of how their work will be impacted and how the change will help to fulfill their purpose. An option for organizational assessment for readiness for change is the use of a tool developed specifically for the purpose of assessing organizations prior to the change. Readiness assessment tools are used to bridge the gap between theory and the actual factors of a particular implementation (Miake-Lye et al., 2020).
2. Determine the ultimate goals for your DNP project.
The ultimate purpose of this is to reduce 30-day readmissions for patients living with chronic obstructive pulmonary disease through education during their initial hospitalization and follow-up after discharge. The hope is that through the implementation of a discharge bundle to improve education and support post-discharge, patients will be able to remain at home and better maintain their health thus improving their quality of life. The purpose of the discharge bundle is to empower the patient and engage them in their own disease management so they can recognize when they might be getting into trouble and take action before it is bad enough that they end up back in the ER. The stakeholders for the change project are the selected patient population and the nurses on the selected unit.
3. Describe how your project goals relate to the organization’s mission and vision.
The goal of reducing the readmission rate is in alignment with the organization’s mission and vision. The mission statement for Prisma Health is to inspire health, serve with compassion, and be the difference. The purposed change project aligns with the elements of inspiring health and being the difference for that particular set of patients by helping them to preserve as much of their health as possible.
