The object of this paper is to have you create a useful, concise care coordination plan for a client in the ambulatory setting. For this purpose, you will develop a full picture of the client, identify two issues, and write SMART goals for those issues.
You may choose one of the clients below or create your own.
All clients are in the ambulatory care setting, not hospital, not ED.
Paper maximum: two pages not including references or title page
Minimum of two scholarly sources as evidence cited for your SMART goals (peer-reviewed journal or professional publication) and no older than six years. Web sites and consumer sources may be additional, but not in place of scholarly sources.
