Case Study, Chapter 7, Grand Nursing Theories Based on Human Needs
Carrey Houghton, 24 years old, has just been admitted to the nursing unit with a temp of 101.8, shaking, chills, difficulty breathing, loss of smell and taste, and abdominal discomfort. She denies having been vaccinated. Her intended husband, Gregg, states that they attended a motorcycle rally in a small town in their state two weeks ago. He tells the nurse that approximately 1,000 bikers attended the rally; no one was wearing masks at the rally or in the bars and dance venues, so they did not either.
Geri George, RN, CNS, does a thorough assessment and finds crackles and rhonchi over most of the lung fields with distant breath sounds over both bases. Nurse Geri has Carrey cough, collects a sputum specimen, and then swabs her nose for a COVID examination. Carrey complains of tenderness over the left, upper abdomen with palpation. Denies any urinary problems, so Geri collects a clean voided urine specimen.
The nasal swab comes back positive for the COVID Delta variant. Sputum and urine are also positive for the Delta variant.
The physician starts Carrey on oxygen by nasal cannula, medication for pain, and intubation and respiration support on standby.
She then starts Carrey on a series of injectable medications (Remdesivir) and an injection of monoclonal antibody that will help build up her immune system.
