This case study will give you the opportunity to work through a potential workplace challenge. Use the readings in Schmidt and Brown to respond to this case study.
A nurse manager in an acute care agency brings a nursing challenge to the EBP, Research, and QI Committee for assistance. The challenge is based upon a problem with accurate temperature assessment for sepsis identification.
Infection control nurses currently follow recently added sepsis identification guidelines. Subsequent to research and discussion with medical practitioners, a guideline and order set was developed to help identify patients who are developing sepsis; these patients urgently need early identification to prevent complications. Patient’s temperatures are checked at four-hour intervals, as patients showing signs of hypothermia (temperature under 96.4 degrees) are at greater risk of sepsis. A low temperature reading triggers an immediate response to document sepsis and initiate early treatment.
However, too many patients have triggered this response (due to a recorded temperature of under 96.4 degrees). When rechecked, many of these temperatures were actually much higher. This situation has created excessive work for the nurses and practitioners as they must sort out who is actually at risk of sepsis and recheck all of the patient’s vital signs.
As a member of the EBP, Research, and QI Committee, you must decide how to tackle this problem.

As the EBP, Research, and QI Committee gets started, one of the committee’s first tasks is to decide whether this meets the criteria for either a QI project or an EBP change. Identify which process would be best for this challenge, and explain your choice. Identify at least one resource other than the text to support your decision.
Discuss why this would not meet the criteria for a research study.
Identify a possible clinical question that will guide this project. Use the PICOT process identified in Schmidt and Brown, section 3.3. Keep in mind that your question may not need all of the PICOT identifiers.
Regardless of whether you’re using EBP or QI, you must be able to access the literature around that topic. What databases will you search, and what keywords would you use?
Your paper should be between 500 and 800 words (2 to 3 pages). Use proper APA style for all references. Be sure to proofread your assignment before submission for overall organization and clarity and proper grammar, spelling, and punctuation.
