Write a Memo from the Quality Improvement
(QI) team to the Executive Team of the Primary Care Clinic
Scenario: At your site of practice in a primary
care clinic, you are leading the Quality Improvement (QI) Team in planning
a Quality Improvement (QI) initiative aligned with current or emerging national
quality priorities and with the intention to incorporate relevant standard
quality performance measures to support possible benchmarking. You have been asked to inform the Executive Team of
the Quality Improvement (QI) initiative measures plan and
needs. On behalf of the QI Team, post to the discussion board a Memo
from QI Team to the (practice site) Executive Team.
Please respond to prompt #1-4 in approximately 150 words each. The last prompt
#5 should be 200-300 words or more. 1. The QI
focus (goal) – weave in own definitions of
value-based care and benchmarking
2. The set
of measures chosen by the QI Team (minimum of three [3]) and the fit
(how/why) of each to which priority areas of the Centers for
Medicare & Medicaid Services (CMS) meaningful measures initiative
(MMI). a. Reasons
can be from published evidence and/or from intuitive logic. Measures can
be from the CMS Measures Inventory Tool (CMIT) (active, development, or
pending) and/or you can suggest measures not in the inventory. Include at least
one measure that is patient-reported.
3. The ‘type’ each
chosen measure represents (using the CMIT ‘Type’ categories. The CMIT is a
repository of >100 measures and each is associated with at least one CMS
initiative), and why important to the QIT initiative.
4. Projected
key project measurement implementation challenges with request
for at least two organizational/systems supports for and
why/how important.
5. For
memo closure, besides referencing any sources use in the text, cite at least
one reference related to measurement for your QI focus and briefly annotate it
with what you most learned from it. (Unlimited word count for #5)
