Writing a soap note on the patient. Please read on the patient information from what I have. The main thing to do is Only make correction and add information for the diagnosis part #3, hypotension, why it is rule out. Then find the treatment plan for benign paroxysmal vertigo (education on manuever etc). Otitits media medication etc
Please use evidence base reference within 5 years. Example for the plan/treatment:
1.Sinusitis (J32.9) and allergy rhinitis:
A: Augmentin 875 mg PO BID x 7 days
B:Flonase 2 spray/nostril Daily
C:Zyrtec 10 mg PO Daily
D:Drink plenty of fluids and get adequate rest
E:Continue healthy lifestyle
F: Nasal spray (neti pot) QHS
G: Do not drink alcohol while on antibiotics
H: Return to clinic in 1 week for follow-up
I:Consider ENT referral if no improvement
J: Return to clinic if symptoms worsen or persists, or for any other concerns
2.HTN (I11.0)
Prescribe Amlodipine 10 mg PO Daily
Monitor blood pressure. 3 times a week at home
Re-check BP at next follow-up visit
